Prevention Assessment Team (PAT) - Drop-In

The Prevention Assessment Team (PAT) will be at Horsham Library 1st Wednesday of the month between 10am-1pm, offering advice and information to help residents access the support they need to live independently at home and enjoy life as part of their community.

Come speak to our friendly multi-disciplinary team of Community Prevention Officers, NHS Nurse Advisors, and Age UK WSBH Community Support Workers. 

Our nurses are also offering Blood Pressure checks (not available on 1st September). We are also offering bookable appointments during this time for 1-2-1 support.
